Completion of the form below is the first step in the process.
Once you have submitted the form below, you will receive an automated email reply which includes additional information and forms that you will need to complete and return to finalize the approval process.
Your submission will be reviewed for approval by our onboarding staff, and you will be notified via email of the outcome. Approval can take 3-4 days, based on the accuracy of the documents submitted.
We offer a 10% discount on purchases with free shipping within the contiguous U.S.
“MOQ” (minimum order quantity) is $1,000usd for wholesale customers.
A Minimum Advertised Price (MAP) policy of no less than 10% of product MSRP is enforced.
Additional discounts are available in the following pricing tiers, with a max wholesale discount of 20%

Your Proforma Invoice will include links for payment via credit card and ACH bank transfer. Payment must be received and verified before your order is shipped. No Exceptions!
Lead time for order shipments varies by shipment size, but orders are usually shipped within 48hrs of payment receipt during normal business hours.
We ship with FedEx Ground and LTL services.
Amazon or other marketplace wholesalers that require a Letter of Authorization (LOA) must complete and submit our LOA information form (link provided), and email it to [email protected]. Attention: LOA Form Request. Upon receipt, our onboarding staff will transfer your company details to our letterhead and email it back to you. This can take up to 3 – 5 business days.
In addition, Amazon resellers are asked to submit their merchant token to [email protected]. Attention: Brand Registry / Merchant Token. This allows us to add your store to our Amazon Brand Registry. This should decrease the likelihood of any "takedown issues" in regard to Zion Health products sold on Amazon. Though there is no "guarantee". We have no control over Amazon's takedown policy and can only provide minimal assistance with such matters.
Zion Health does not provide dropship service for Amazon FBA customers, or do any special labeling or packaging.
Returns are accepted for damaged products only, and we offer credits towards your next purchase. A 20% restocking fee will be applied to to all other returns, and credit towards your next purchase will be issued. You are responsible for shipping the returned products back to our warehouse at: Zion Health Inc., 430 E Grand Avenue, S. San Francisco, CA 94080.
